  1. Please fill out the following form in full. It will be emailed to Discovery Child Care Centre and your child's name will be added to our waiting list. You must also print out the form that is produced after pressing "Submit" and mail it with your $25/family payment to the Centre within 7 days of submitting your request.   2. Please note, if you must fill out a form for each child you are requesting to be placed on the waiting list, however only one $25.00 fee is required for each family.
