Child Care Survey

Dear parents/guardians,

One of our goals is to provide high quality, educational programs where children learn through play. We are constantly planning and evaluating the programs that the children are involved in. With this in mind, we have developed a survey so that we may receive feedback from you. Some of the areas dealt with are: program, staff, environment, and communication. We have provided a space at the end of this survey for any thing else you would like us to know. We know that your time is very valuable and we have tried to keep the survey short and to the point. All surveys are anonymous if you do not fill out your name. That being said, we would love the opportunity to discuss any concerns, ideas or suggestions that you may have. If you leave your name we will respond to your comments in a timely manner.
